
Information for State and Local Health Departments
The CDC Foundation is the nonprofit partner of the Centers for Disease Control and Prevention (CDC). CDC director, Dr. Julie Gerberding, asked the CDC Foundation to activate our Emergency Preparedness & Response Fund to provide flexible, immediate resources for CDC and its public health partners working on the frontlines in response to Hurricane Ike. Many individuals and private organizations have contributed to this Fund to support the public health community as they respond to this enormous public health challenge.
State and local health departments may apply to the CDC Foundation for assistance in meeting public health needs related to Hurricane Ike. Preference will be given to health departments in those states immediately affected by Hurricane Ike.
Eligibility
Only state and local public health departments affected by Hurricane Ike are eligible to receive funds. Individuals, nonprofit organizations, non-governmental organizations or other government agencies not focused on public health are not eligible.
Process
- Download and complete the request form (Word document).
- E-mail your completed form to lmcgehee@cdcfoundation.org and copy eocfoundation@cdc.gov
- The CDC Foundation will work with the public health experts at CDC’s Marcus Emergency Operations Center to evaluate and prioritize each request for funding. The CDC Foundation will make all final decisions about the distribution of funds.
- The CDC Foundation will respond to your request within two (2) business days.
